Studio Policies

STUDIO INFORMATION
Location: 49 W. 24th Street 4th @ Singer's Forum (at 6th ave)

Payment information: $110/hr. $65/30 minutes. Cash/Check.
Insurance: I do not currently accept insurance for voice therapy, however, upon request I will provide you with invoice and session notes that you may submit to your insurance company.

Scheduling Procedures

* Go to www.tomburkevoice.com
*Click schedule/policies and follow the links to schedule either a 30 or 60 min lesson.
* New students please schedule a 1 hour lesson for your first time.
*To schedule multiple sessions, you will need to hit "refresh" on your * browser after confirmation of each appt.
*Set up an account to monitor your reservations.


Voice Therapy

* Initial Evaluation is 1 hour
* Follow-up visits are 30 minutes.
* "Repertoire Integration" may be added for additional 30 minutes.
* When available, please bring a referral/prescription from your physician, list of medications, reports and any photos/videos of your larynx.
* I do not currently accept insurance for voice therapy, however, upon request I will provide you with an invoice and session notes upon completion of therapy that you may submit to your insurance company.
* Contact your insurance company to determine if pre-certification is necessary for the following codes: Speech/Voice Evaluation: 92506 and Voice Therapy: 92507.

Cancellation Policy:
Please allow 24 hours notice for cancellations via text message or voicemail only please. I will require payment for cancellations provided less than 24 hours. This includes illness, auditions, etc.

Recommendations:
Please bring a method of recording our session (i.e. Tape recorder or iPod) I have an iTalk to use for your iPod video if available. Videotaping is also possible and can be transferred to your laptop via USB if you'd like. While I am an accompanist and can offer performance coaching, I do not specialize in repertoire coaching. If you require rep for building your book, I'd be happy to recommend you to one of my colleagues.
